Why medication reviews?
Each year, approximately 230,000 Australians are admitted into hospital as a result of medication misadventure, costing our national health system approximately $1.2 billion anually.
Medication reviews are a comprehensive and analytical investigation of an individual's medication regimen, performed by highly trained, accredited pharmacists.
The ultimate purpose of a medication review is to optimise medication management, improve adherence and reduce risk of misadventure through increasing a patient's understanding of their medication, and collaborating with their GP (and other health professionals involved in their care) by providing advice and recommendations concerning their medication.
At Northern Sydney Medication Review (NSMR), our goal is to minimise not only the risk of misadventure, but also our patients' anxieties regarding ever-increasing pill burdens, thus maximising adherence to appropriate medicines. We do this through a meticulous process of patient counselling, upskilling and engagement in a team-care approach.
