Supporting GPs with Efficient HMR Services

Clinical Pharmacist SydneyNorthern Sydney Medication Review (NSMR) works closely with general practitioners (GPs) and medical practices to facilitate seamless Home Medication Review (HMR) referrals. NSMR provides a reliable and professional service where GPs can confidently forward referrals, knowing their patients will receive a thorough and patient-focused medication review.

A Seamless Referral Process

GPs are crucial in identifying patients who would benefit from HMR. By partnering with NSMR, GPs and medical practices have a trusted and efficient service to forward HMR
referrals, ensuring patients receive the best possible medication management support.

Key aspects of our referral process include:
  • Dedicated Referral Handling: Referrals are managed efficiently, ensuring a smooth transition from GP to pharmacist.
  • Comprehensive Medication Reviews: Thorough reviews to improve patient adherence and safety are conducted by an experienced pharmacist.
  • Ongoing Collaboration: Clear communication with GPs, pharmacists, and patients for optimal care coordination.
  • Simplified Workflow: A streamlined process to help reduce the administrative burden on GPs and practice staff.

Where can I send the HMR referral?

Referrals can either be emailed as an attachment to richard@nsmr.com.au or faxed to 02 8076 1523.

What information should I send with the referral?
Best Practice and Medical Director have templates for MBS 900 that can be pre-populated with the relevant information. Typically, this will include the patient’s name, DOB, address, contact information, medication list and medical history.
Additionally, any relevant pathology that can be provided will also be extremely helpful.
How is my patient contacted?
I shall contact your patient directly to arrange a time. Thus, please ensure there is a current contact number on the referral, and that the patient is also informed that I will be reaching out to them (this is to minimise the risk of your patient thinking that the appointment call is from a cold-calling telemarketer).
What is the usual turnaround time for the review?
After your patient has been seen, the report should typically be sent to you within a week. I will usually send the reports via email. However, if you have a different preference, this can be indicated on the referral.
